Who can Benefit

Here is what you need to do

  1. Fill out the Application. All of your information will be kept confidential
  2. Along with your completed Application you must provide: copies of your most recent pay stubs and anyone else who contributes to the household income; and a copy of the first two pages of your child's most recent Individual Education Plan(IEP). * If you are purchasing equipment or services, please provide a copy of an invoice or estimate for the service along with your Application. ** Applicants that don't have an IEP may substitute a doctor's letter or script explaining their disability.
  3. Mail your completed Application and materials to:

  4. A volunteer board member from the Zane's Foundation, Inc. will contact you to review your Application and paperwork.
  5. You will receive the decision regarding your grant. The Board of Directors review Applications on a monthly basis.
